Position Purpose: Provide telephonic in-home disease management services to individuals with diabetes. Services include educating members with diabetes about the disease and assisting with dietary needs.
Knowledge/Experience: Bachelor’s degree in related field. At least 5 years of direct patient care to individuals with diabetes. Outpatient or long-term education and lifestyle management coaching experience preferred. Background in nutrition a plus.
Position Responsibilities: Act as the Health Coach for patients with diabetes • Meet minimum standards for productivity and clinical results • Provide assessments on new participants to determine clinical risk and readiness to change • Maintain accurate and timely documentation in member management software of telephonic and written communications • Act as case manager when integrating with other healthcare service vendors as specified by individual clients • Make calls to newsletter participants/members as individual client needs dictate • Complete required reports in a timely manner • Enhance clinical and counseling knowledge by attending and actively participating in Company training sessions • Attend relevant professional seminars to maintain licensure/certification • Comply with Company policies and procedures including handling confidential forms and accreditation standards.

Position Purpose: Perform in-home respiratory assessments to patients and educate them on the physician’s treatment plan and medication regimen.
Knowledge/Experience: Bilingual candidate a plus. Associate’s degree in Respiratory Therapy or graduate of a two year American Medical Association (AMA) approved Respiratory Program. 1- 2 years practical experience.
Position Responsibilities: Plan, perform and document the delivery of patient education services in accordance with policies and procedures • Perform the initial respiratory assessment, environmental assessment, education presentation and basic respiratory questionnaire • Perform follow-up assessment and presentation of the physician’s treatment plan and medication regimen to patients • May be required to lift up to 45lbs. on a regular basis • Able to travel up to 250 miles for patient visits.
Licenses/Certifications: Registered Respiratory Therapist (RRT) or Certified Respiratory Therapist (CRT) as recognized by the National Board of Respiratory Care (NBRC) and licensed by the work-site state as a Respiratory Care Practitioner. Current CPR certification.

Position Purpose: Provide telephonic disease management services to individuals within a specific disease category. Services may include educating members, counseling and coordinating care.
Knowledge/Experience: Registered Clinical Exercise Physiologist, Registered Exercise Specialist, or Exercise Physiologist currently eligible to sit for one of the above ACSM exams. Master’s degree in Exercise Physiology preferred. Minimum 3 years experience in clinical physical rehabilitation and/or hospital setting, including patient education, specialized exercise programming and counseling. Experience with back pain and/or sports medicine essential.
Position Responsibilities: Act as the Health Coach for patients within a specific disease category (diabetes, CAD, heart failure) • Meet minimum standards for productivity and clinical results • Provide assessments on new participants to determine clinical risk and readiness to change • Maintain accurate and timely documentation in member management software of telephonic and written communications • Act as case manager when integrating with other healthcare service vendors as specified by individual clients • Make calls to newsletter participants/members as individual client needs dictate • Complete required reports in a timely manner • Enhance clinical and counseling knowledge by attending and actively participating in Company training sessions • Attend relevant professional seminars to maintain licensure/certification • Comply with Company policies and procedures including handling confidential forms and accreditation standards.

Position Responsibilities: Act as the Health Coach for patients with hypertension, hyperlipidemia and other associated cardiac risk factors • Meet minimum standards for productivity and clinical results • Provide assessments on new participants to determine clinical risk and readiness to change • Maintain accurate and timely documentation in member management software of telephonic and written communications • Act as case manager when integrating with other healthcare service vendors as specified by individual clients • Make calls to newsletter participants/members as individual client needs dictate • Complete required reports in a timely manner • Enhance clinical and counseling knowledge by attending and actively participating in Company training sessions • Attend relevant professional seminars to maintain licensure/certification • Comply with Company policies and procedures including handling confidential forms and accreditation standards.

Position Purpose: Contact disease management program candidates to determine eligibility, provide general information, enroll participants, and complete first phase assessment calls. Assist with provider communications and clinical data collection. May provide tobacco cessation counseling.
Knowledge/Experience: Bachelor’s degree in Exercise Physiology, Nutrition or related health field or Licensed Practical Nurse (LPN). 2+ years of related customer service experience.
Position Responsibilities: Make initial call to disease management candidates to determine medical and administrative eligibility, level of interest and provide general information about services offered • Conduct first phase assessment calls on participants who elect to enroll in telephonic counseling • Act as a Tobacco Cessation counselor • Contact participants enrolled in the TransFormations program and if interested enroll them in active counseling and schedule assessment call • Enroll candidates who seek to enroll by Company website, fax transmission, or case management referral • Enter pertinent information into Company database • Assist with clinical data collection from providers.
Licenses/Certifications: Requires Bachelor’s degree in health field or LPN.
