Health management is an approach to proactively impact the health status of individuals and reduce healthcare costs. Nurtur Wellbeing is a family of health management and wellness programs designed to address the health needs of all members of a population.
Nurtur Wellbeing includes four lines of programs to give plan sponsors the tools they need to create a philosophy of wellness and develop a culture of health. Our platform for wellness and online health management is developed in an exclusive partnership with Duke Medicine, a globally-recognized leader in health and wellness research. Our marketing collaboration provides members with access to onsite programs for Diet & Fitness, Executive Health, Integrative Medicine and Weight Loss Surgery.

Wellness Tools
Individuals can only be expected to make educated health decisions if they are empowered with information. Awareness is all about providing people with the tools and resources they need to be informed. Tools such as health risk assessments and biometric screenings are combined with online health management, incentive management and 24-hour nurse line resources to reinforce positive lifestyle behaviors. Together, these supports allow individuals to stay fit, manage stress and self-advocate for their personal health. By promoting Awareness, plan sponsors can reduce overall healthcare costs and support a culture of health.
The central component of Nurtur Awareness is The Personal Health Development (PHD) Network. The PHD Network is an innovative online health management system that leverages interactive HRA and biometric data to identify individual health risks and modifiable lifestyle issues related to a variety of health concerns including the onset of heart disease, diabetes or stroke. Personalized lesson plans are designed to help people reach a “readiness-to-change” mindset and make positive lifestyle changes to improve their overall health.

Wellness Programs
Guidance programs are designed to mitigate risk factors and prevent the development of chronic disease. Wellness programs include Lifestyle Management to help people who use tobacco products or have high blood pressure, high cholesterol, poor diet, not enough physical exercise or elevated stress to start living healthier. This approach combines telephonic lifestyle coaching with online lessons, and educational materials to promote positive behaviors and reduce health risks. Guidance also provides stand-alone programs to address many specific health concerns.

stress, tobacco cessation and weight management]

Disease Management
Motivation disease management programs support, encourage and inspire people with chronic conditions to take stock of their health and change their lives for the better. The reality of living with back pain, depression, anxiety, substance abuse, diabetes, heart disease or respiratory disease can be overwhelming. Often people don’t understand what can happen to them if they don’t manage their conditions – it’s not real until it happens. Nurtur Health Coaches connect with people and provide caring support to make the risks real – before they happen. They work with individuals to develop customized care plans and set goals they want to achieve. With Motivation, people successfully transform their lives to become active self-managers of their health.

- Back Pain
- Depression
- Diabetes [including type I and type II diabetes]
- Heart Disease [including coronary artery disease (CAD) and heart failure (HF)]
- Respiratory Disease [including asthma and chronic obstructive pulmonary disease (COPD)]

Episodic/Catastrophic Care Management
For individuals faced with complex health issues or unexpected medical emergencies, Support programs provide information, guidance and resources to improve health outcomes and avert complications. Telecare Management offers a technology-enabled model of care to provide remote monitoring and real-time communication for patients diagnosed with chronic disease such as diabetes, heart failure and chronic obstructive pulmonary disease (COPD). Support also provides programs for individuals with episodic or catastrophic health issues such as high-risk maternity or cancer to help them better understand treatment options, navigate the healthcare system and advocate for their own care.
