After learning about an incentive program, John and Monica both enrolled in health and lifestyle management. They each began to see improvements in just a few short months working with their health coach, Sara.

Problem: Tobacco Cessation
Age: 64

A three-pack-a-day smoker, starting at age 13 John was ready to quit. He tried unsuccessfully to quit in the past, but recent events in his personal life along with the increasing cost of cigarettes prompted him to make a lifestyle change. With the help of his Health Coach, Pam, John has been smoke-free for seven weeks and has no intention of starting again.

Problem: Depression
Age: 40

Jane was going through a tough time in her personal life. After a health assessment at work, she was contacted by Health Coach, Abby. From the beginning Abby was able to provide Jane with helpful information and put her in touch with valuable resources to help her cope with her depression.

“I can be honest with Abby and tell her about what I’m going through. It’s funny to think that I can be so comfortable talking with someone that I’ve never actually met. Just knowing that she is always there to talk to is very reassuring.”
Problem: Type-2 Diabetes
Age: 63

Like many people with chronic health issues, Tom was leading a fairly normal life when he was first diagnosed with Type-2 diabetes. His job was very challenging and often stressful with frequent travel. He wasn’t sure how to manage his health issues. Today, he has successfully lost weight and his A1C is back within normal range. Tom credits his Health Management Program and Health Coach, Betsy G., with much of his success in managing his diabetes.

Problem: Asthma
Age: 36

Both Michelle and her son, Nelson, have asthma. Before participating in the Nurtur Respiratory Disease Health Management program, they lived with the fear of being short of breath. Today life is a lot less scary. Michelle has learned from her Health Coach, Cheryl, how to track her breathing with a peak flow meter and manage flare-ups before they get out of control.

“It’s been great to talk with someone who is so helpful in finding solutions that work for us and our lifestyle. Cheryl has provided us with the knowledge of how to live with asthma in a healthy way, and how to live without being scared of it.”
Problem: Stress
Age: 20-something

When Stacey’s husband made a career move, it meant moving her family, including an infant and toddler, across the country. Settling into their new home in an unfamiliar community was overwhelming. She called the Employee Assistance Program offered through her husband’s workplace to talk about her frustrations and need to find reliable childcare services. It was such a helpful experience that she credits the program for freeing up the time for her to land her new job.

Problem: Heart Disease
Age: 61

For a long time, Spence did not consider himself to be a healthy person. He was overweight and had three coronary stents to treat blocked arteries. When he made the decision to actively improve his health, he turned to his Health Coach, Jen F. for help. Together they came up with practical ideas that Spence could commit to achieving, like cutting back on his soda intake from 5 cans a day to 4 cans a week.

Problem: Back Pain
Age: 38

Kirsten, a busy mother of two, needed help managing the chronic back pain she had been living with on and off for fifteen years. The physical demands of her life included taking care of her young children, running her own gardening business, and tending to her horses and five acres of land. With support from her Health Coach, Jen F., Kirsten is able to be active without pain and fear.

Problem: Type-1 Diabetes
Age: 52

After suffering a dangerous and frightening episode of hypoglycemic unawareness, Joanille called her Nurtur Health Coach, Betsy G., for help. Betsy quickly went to work and arranged a conference call with Joanille and two other diabetes specialists. The team gave Joanille their time, support and insight when she needed it most.
